A click is counted when a visitor lands on your Link in Bio page, and again each time they tap one of the links on it. For example, if someone visits your page and taps two links, that counts as three clicks in total.

Yes. If you've connected a custom domain to your Nimble Links account, you can use it for your link-in-bio page just like any other link.
